Appointments
Appointments to carriers for the sale of insurance products with Long-Term Care features involve a few extra steps. For all carriers you will need to do the following:
- Review the rules for the state in which the client lives – that is your application state. A good tool for discovering what each state requires can be found here this link also provides names of vendors who offer Long-Term Care CE recognized by ALL LTC carriers.
- For all states, we will need your original certificate of completion for the initial 8 hours of LTC Continuing Education.
- We will need the certificate for your most recent refresher course. Most states require a 4-hour refresher course every two years but some states vary (See VA and CA for example in the map linked above).
- We submit appointment paperwork for any case involving LTC in advance of the application signatures.
Many LTC carriers have a different way of submitting contracting paperwork and appointment requests. Here is the current information for assuring we have the right paperwork to process your request for appointment:
- Life with Long-Term Care riders (Axa, John Hancock, Nationwide, etc.) – this is processed the same way any life carrier appointment request is handled. Please refer to our contracting page for additional details but also submit your LTC CE certificates as mentioned above.
- Asset-Based LTC for Lincoln and Nationwide Care Matters – see point one.
- Asset-Based LTC with One America – this carrier has independent paperwork requirements. You can download the paperwork for individual contracting and corporate contracting. Please keep in mind that if your business is licensed to sell insurance it can be paid for this business but you will need to submit both the individual contracting AND the corporate contracting.
- Traditional LTC – Our vendor for traditional LTC applications accepts the same packet of information we use to process the majority of our insurance business. If you have not already submitted that packet download it here.

Case Submissions
Nearly all cases with an LTC component qualify for the Kelle App service with the exception of traditional LTC. For traditional LTC submissions, we HIGHLY recommend the Application Partner program available through LTCI Partners. Both of these services are available to you at no charge and provide an In Good Order application process for your clients.
To set up an Application Partner app for traditional Long-Term Care complete this request form and return it to Kelle along with the illustration for the case.
To submit cases without using the Kelle App service please follow these steps:
- Lincoln Money Guard – use our forms engine (left navigation) to request the quick ticket.
- Nationwide Care Matters – use iGo (left navigation) and for product type select Life/LTC
- One America – use iGo (left Navigation) and select either Asset Based LTC Whole Life or, if the case is an annuity case, Asset Based LTC Annuity
- For all other cases – use iGo and select the base product (ex. universal life). Be sure to indicate where asked that this case includes an LTC rider.
